Dr. Gregory Echt explains how oncologists arrive at a cancer diagnosis

Irving, TX – Prostate cancer is the most common type of cancer affecting men, with estimates stating that about 1 in 6 men will experience it in their lifetime. Previously patients would go to hospital when the cancer had already metastasized to other parts such as bones. Patients would also have developed other symptoms or they simply came in for other problems such as urinary issues. This caused quite a high number of deaths from prostate cancer. Statistics show that the numbers have fallen by 33% over the past decade due to earlier detection and better intervention methods once diagnosis was arrived at.

“Technology has made quite the difference over time and has improved survival rates,” says Dr. Echt. “Diagnosis remains the beginning of this journey.”

Diagnosing prostate cancer is only done through a biopsy. A biopsy is the removal of tissue samples by an urologist. A pathologist then studies the tissue for signs of cancer and assigns it a Gleason Score depending on the cell patterns found. This score is graded based on the following observations:

• Gleason Score of 1 – This means that the pathologist found cancerous tissues that look like normal cells. The tumor in this situation is low-grade and is less likely to grow and spread fast compared to those with higher scores.

• Gleason Score of 2 to 4 – Some cells look like normal and healthy prostate tissue while others do not. This means that the tissue showed more than one cell pattern.

• Gleason Score of 5 – This indicates that the cells appeared abnormal. It shows that they are also possibly haphazardly spread throughout the prostate. This type of score is an indication of a high-grade prostate cancer. Chances of this spreading rapidly are very high.

Where the pathologist finds a single pattern the score is doubled. Where there is more than one pattern, the most common patterns are graded between 1and 5 and added to each other.

• The total should be a number between 2 and 10 where 2 is a low-grade, low-risk tumor and 10 is a high-grade, high-risk tumor.

“The Gleason Score then determines what treatment course the patient will receive,” adds Dr. Echt.
