Hospice of the Calumet Area Assists Ill Patients with Transitions Program

Hospice of the Calumet Area's, non-profit organization, Transitions Program offers support services as simple as help with daily routines and as complex as decisions with care options.
 
[MUNSTER, 11/6/2017] – Hospice of the Calumet Area improves the quality of life of individuals struggling with serious illnesses. The hospice provides compassionate care for patients “ ...facing a life-limiting illness” and their families.

For the organization, the basic worries of life (e.g., running errands, cooking, and housework) can be complicated for a number of terminally ill patients. The hospice offers relief by providing assistance in the form of their Transitions program.

Easier Transition

The center’s support team brings relief by offering companionship, assistance with tasks, and referrals to community resources. Funded by the community’s resources, the Hospice provides services at no cost.

Some of the services provided by the staff include the following:

• Help in making decisions about care options
• Connecting individuals with the right resources for home assistance
• Support in coping with challenges due to life-limiting illness

Volunteers also provide companionship visits, as well as assist patients with errands and meal preparations. Caregivers also receive respite breaks.

A Different Approach

The hospice applies a family-centered approach, which includes dedicated participation from their team of doctors, nurses, social workers, trained volunteers, and counselors.

Each staff member will address all the needs of the patients: physical, spiritual, and emotional. The Hospice’s primary goal is simple: to keep patients free from pain and comfortable in familiar surroundings.

The center remarks, “We cherish life at a time when others may feel like giving up.”
